Therapy sessions:
$190: 60 minutes
$175: 45 minutes
Insurance :
Dr. Saxby is currently in-network with Aetna in Tennessee and their EAP.
Dr. Shollenbarger is currently in-network with Optum/ UBH.
We are adding many more insurance plans, but if we are currently out of network with your plan, some insurance plans will cover a portion of the session fee, once your deductible has been met. How it works is this: you will pay your psychologist directly for your session and, if you’d like to be reimbursed by your insurance, we will provide you a receipt to submit to your insurance company.
If you would like to investigate the possibility of reimbursement for out-of-network coverage, please check your policy carefully and ask the following questions of your insurance provider:
-Do I have behavioral health or mental health benefits? Does my particular plan have an out-of-network benefit?
-Do I have a deductible, how much is it, and has it been met?
-After my deductible is met, what percentage of the fee does my plan cover?
-How many mental health sessions per calendar year does my insurance plan cover?
-Do I need an authorization to begin outpatient therapy?
-Is approval required from my primary care physician?
-How do I access the form(s) needed to submit a request for reimbursement?
Fee changes:
We will periodically raise our fees with 3 months advance notice.
No Surprises Act: Good Faith Estimate
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees. Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
Cancellation & Missed Appointment Policy
Scheduled appointments that are not cancelled within 48 hours or that are missed will be charged the full session fee.
Payment
Payment for services is an important part of any professional relationship. Payment for our services is due at each session in full and if we are in-network, you co-payment is due at the time of your session. We will provide you with a receipt for services, which you may be able to submit to your insurance for out-of-network reimbursement. You can use money from all three types of health spending accounts – health savings accounts (HSAs); health reimbursement arrangements (HRAs); and flexible spending accounts (FSAs) – to pay for sessions. Oftentimes, such health spending accounts have debit cards that can be used to pay for services. You can also pay with cash or your personal credit/debit card and have your health spending account reimburse you. Please note that it is always your responsibility to pay our full agreed upon fee or insurance co-pay for services at the time they are rendered.
